Please complete this form and submit it to us at least a week prior to your desired consultation time.  We will make every effort to work with your requested date/time.

Go to the Spiritual Instruction link on this website and read The Sacred Circle.  This very important educational material prepares you for our time together by explaining the process of working with a medium and the World of Spirit so that you will have the best experience possible.  If you wish to meet with your Angel Band, please also read Your Angel Band.  For phone consultations, also read Connecting With Heaven and Earth (a meditation to relax and prepare you, and make attunement easier).  Practice the meditation approximately 10-15 minutes before your phone consultation.  Be prepared with any questions, and open to what comes from Spirit.  Please avail yourself of all the offerings in the Spiritual Instruction Section on Mariah's website.

You may choose to have Mariah record your session on a digital voice recorder and prepare a CD for you.

Payment forms for in-person and phone consultations: PayPal or certified check. Mariah does not accept credit cards, but PayPal allows the use of credit cards or your bank account. Please check Fee Schedule to include cost of additional family or friends joining us in person or by phone for a consultation.

Phone consultations: Payment must occur prior to a phone consultation to reserve your appointment time. The following forms of payment are accepted: PayPal, certified check - no personal checks or credit cards (PayPal allows payment via credit cards or your bank account). If you are paying via certified check, an appointment will be scheduled when the certified check arrives. If the technology is available for all concerned, family/friends can participate in a conference call by phone or Skype. See my Fee Schedule for cost of additional people. I offer Skype phone consults. To have a Skype phone consultation you need: a webcam (your computer may have one built in) and either a headset or a microphone and speakers. Skype calls are free from Skype member to Skype member, and the software is free to download at www.skype.com. It adds a lovely, personal connection in a rather Star Trekish way! In "type of consultation" box below just select Skype.

No matter where you are in the world, Spirit will bring upliftment, joy, courage, encouragement, guidance, clarity and love, for Spirit knows no boundaries.  It is Mariah's joy and honor to be a channel for you, and for all who love and care about you in the Land of Light.

      

      

Please provide only very basic information for your focuses.  Examples:

  • Relationship issues: Please give names and/or relationships of those to be discussed, ie: husband (Mark); boss (Jane), etc

  • Mediumship: Prior to your phone consult, Mariah will offer a prayer of protection, guidance, clarity and whatever else may be needed.  She will invite your family and friends (both human and animal) and your Guides from the Land of Light to come for a visit at the appointed time. If you wish only one particular person to visit from the World of Spirit, and that spirit person is the focus of your session, Mariah wlll need to know who that particular person is: spouse, parent, child, animal, co-worker, friend, etc., and how long they've been in Spirit.  Please do not volunteer any other information, especially not the person's name, how they passed over, etc.  Mariah may need to speak very briefly with you prior to the consult to determine if that spirit person is awake and available.  No medium can promise who will be available to visit from the World of Spirit, so please be flexible and without expectations.  Being open to all friends and family visiting from the Spirit World is sometimes best, for the more experienced family/friends will often assist the newly-arrived spirit person to build up energy to come in and communicate.   More information regarding mediumship readings can be obtained from reading The Sacred Circle, which you can access through the Spiritual Instruction link on Mariah's website.

  • Intuitive Business Consultations: please state what the issues are, giving only basics: a move, taking on or letting go a partner or employee, changes in operations, finances, etc.
